Lifestyle Recommendations and Chronic Conditions in Rural Colorado: Challenges and Opportunities

Overview

This module will explore the challenges and opportunities related to promoting lifestyle recommendations for chronic conditions like diabetes, heart disease and obesity in rural Colorado. Students will gain an understanding of how rurality may impact managing and preventing health conditions and recommendations and strategies to improve diet and physical activity related health outcomes in these areas.

 

Learning Objectives

After completing this module, learners will be able to:

  1. Understand the link between lifestyle recommendations and chronic conditions such as diabetes, heart disease, and obesity, with a specific focus on rural Colorado populations.
  2. Identify diet and physical activity barriers in rural Colorado, including geographic, economic, and cultural challenges.
  3. Explore the role of healthcare professionals in promoting lifestyles to mitigate and manage chronic disease in rural settings.

Associate Professor and Associate Director of Practice Innovation in the Department of Family Medicine at the University of Colorado School of Medicine (CU-SOM)

 

Bonnie Jortberg, PhD, RDN, CDCES, is an Associate Professor and Associate Director of Practice Innovation in the Department of Family Medicine at the University of Colorado School of Medicine (CU-SOM).  She holds a PhD in Human Nutrition, is a Registered Dietitian Nutritionist and a Certified Diabetes Care and Education Specialist. Dr. Jortberg has been at CU-SOM since 1995, starting in the Department of Preventive Medicine as one of the study coordinators for the Diabetes Prevention Program. She has devoted her career to teaching and translational research related to chronic disease prevention and treatment through nutrition and lifestyle interventions.

Robyn Wearner, Sr. Instructor, University of Colorado. As a Registered Dietitian and health services researchers, Robyn has provided primary care teams and patients with learning opportunities to better understand health promoting strategies. Whether addressing weight management and self-management support strategies or tools to manage blood glucose, she is guided by an overall goal to improve health outcomes for individuals and populations.
